Insurance and Patient Information
As a courtesy to you, we will bill your insurance company for services. While most of our office accept a wide variety of insurance plans, it is your responsibility to ensure that the provider you are seeing participates with your insurance plan.
You must bring your insurance card with you because our staff must make a copy of it. The staff will ask you to review and update as necessary all personal information (address, phone number, etc.), and to note any changes in insurance coverage.

All fees must be paid on the day of your visit. This includes co-payments and self-pay fees. We accept cash, checks, and credit cards (Visa, MasterCard, Discover Card, and American Express).
If you are a patient having surgery at any of our sites, please be advised that you will be receiving a separate bill from your anesthesiologist. This bill will be in addition to the bills you receive from your surgeon and the hospital.
Please also note that your insurance may not cover all anesthesia fees, even though fees for your surgeon and the hospital may be covered. In some instances, you may be responsible for paying co-pay and deductible, depending on your insurance policy.
